Hoya ilagiorum is native to the Philippines, specifically found on the island of Luzon.
In its natural habitat, it grows as an epiphyte (on trees) in the wet tropical biome, thriving in the humid, warm conditions of the Philippine forests. It is often found scrambling or climbing, using trees for support as it reaches for dappled sunlight.
